It goes against every logical bone in my body to say this, but change 
the cable.
<Insert ramble about similar but different experience here>

I also had similar problems with a drive that was developing bad 
sectors. I figure it had exceeded the number it could remap and was 
starting to get flaky. Worse still, it was my swap partition.

That was a bugger to track down.
