Funnily enough, this has come up on this list before. There must be an
archive somewhere ... ;-)

I have also once searched this issue on google and the answer is there
too. The simple answer is in an app that nearly every linux user already
has is: gimp. Other than that I believe scribus is good for the purpose.
As noted before, you open the pdf as an image and write over the top of
it in the appropriate place. Save it as ps then do ps2pdf. As said
before, it's fussy to do, but possible if you really have to.

Of course, they're expecting you to print, write and post off the form.
You could also print, write and scan to pdf, or you could do it the hard
way and wow them with your expertise if they're even going to appreciate
it ;-)
