[PLUG-AV] Title sequence...

With ffmpeg it's easy enough to concatenate input .dv files (but not quite
"cat") to produce a single output file. So the intro and credits files are
easily added without any "post processing" as such, it's just past of the
cmd line.

On hindsight I should have done this for the intro loop when I processed the
Luakit talk, just didn't think of it :)
