Just to show that 24 hours after posting to a mailing list all your
problems really *are* solved (even if not by the mailing list), I solved
the PCMCIA -> ISA adapter problem too.

By default the i82365 module of the Linux pcmcia package will only probe
for the chip at 3E0.  By either moving a jumper to the 3E0 position
(which was undocumented), or loading the i82365 module with the
"extra_sockets" parameter set to 1 so that it probes 3E2 also, the
problem is solved.
