[plug] forwarding X11 - KDM

Thanks Scott for pointing me in the right direction.

Surprisingly, my incompetence was not the root cause of the problem in
this instance. Configurations settings between versions of kdmrc have
changed slightly for Debian and others who are using KDE for dumb
terminals out to be aware of this.
diff as follows:
/etc/kde2/kdm/kdmrc
#Whether KDM should listen to XDMCP requests. Default is false for Debian.
Enable=True
/etc/kde3/kdm/kdmrc
#Whether KDM should listen to XDMCP requests. Default is true.
Enable=False

I had the enable line uncommented and the value in the kde3/kdm/kdmrc
file changed to False which disallowed XDMCP requests. Once this was
commented the settings in Xaccess worked as expected.
