Let's see. Aah yes, it was a Lucent modem!
I used the ltmodem-8.26a9 package (this was about a month ago), which (*quick 
Google*) can be grabbed from here:
   http://www.physcip.uni-stuttgart.de/heby/ltmodem/

I basically just followed the instructions within. Just skimming the docs now, 
it was something like:
  sh scanmodem
  ./build_module
  ./build_deb
  dpkg -i ltmodem-2.4.18cel_8.26a9_i386.deb
  utils/checkout

Since I already had ppp set up previously on a serial 33k modem, it was easy 
enough to tweak the connect/init string and modem device of my existing ppp 
config.
