If there was a problem with hardware (disk, bus, etc) some time in the
past (e.g. days, weeks or months ago), the filesystem was probably not
at fault per se -- it just found itself in an impossible situation
because the damage had already been done. People mention SMART and so
on. Might want to check your disk(s)?
