[plug] html code help

Something like :

<form method=post action="....">
<input type="checkbox" name="cisco" value="cisco"> Check this when you've done
it<br>
<input type="submit" value="finished everything">
</form>

You can (obviously) use tables or CSS to tweak the layout, but the above ought
to work as 'proof of concept'.
