I installed Postfix+Courier-IMAP+MySQL (http://www.high5.net/howto/) at a
school (with hundreds of users), combined with Postfix Admin
(http://high5.net/postfixadmin/) which allows the school's admin to
add/remove users, aliases etc etc.  It may not be quite what you need, but
it is really easy to extend with PHP and/or Perl.
