[plug] SNMP tool

mrtg will do all of that, as will cacti and bigsister.
Bigsister can also use agents on *nix/windows machines and can monitor
services/etc
also you have the ability to send alerts... and acknowledge
warning/critical states.
(ie its more than just a grapher)
