Thanks.  It was the driver.  Changing only the driver (from vesa to nv) 
fixed the problem.

The monitor is a ViewSonic G90f+, an 18 month old 19" CRT.  It has never 
been auto-detected by a linux distro.  The frequency range in xorg.conf 
was H:28-51 and V:43-60.  The monitor's range is H:30-97 & V:50-180. 
Normally if the signal is out of range I get a nice warning in the 
middle of a bright red rectangle.

I have a 2-port KVM switch between the monitor and the MX440-SE, but 
even with a direct connection the monitor is not auto-detected.
