You have a disk problem...

It would help if you included the complete message regarding the fault,
not just the last few lines.

The short version, though, is that your disk is not responding properly
any longer, so nothing works.  This is a hardware fault.

Try unplugging and replugging all the cables to the disk, and see if
things improve.

Otherwise, restore from backups to your new disk.
