I don't know if it's any help, but I had this problem recently twice.
The first time was when I was reconfiguring the server, and the mail servers saw my HELO change
twice in 5 minutes (I stuffed up setting the hostname and mailname).
The second time took a _lot_ longer to track down. My server is multihomed, but I only had a reverse
dns on one of the ip's. Intermittently (and without rhyme or reason) exim would send from the second
interface and my mail would be rejected for bad reputation. I bound exim to the first interface,
*and* set an appropriate reverse on the second interface ip address and it's been all good since.
The only way I found out about the first issue was to send a polite e-mail to the owners of one of
the block lists. They kindly scanned the logs and gave me quite a detailed rundown on what had
happened to cause my server to be listed. Very helpful in fact.
The second one took a lot of tcpdump sniffing to figure out what was going on. Had I read the
rejected mail headers properly it would have happened a _lot_ faster :)
