After some more testing I seem to have found the problem. I was calling 
mpg321 with the entire music list (4500 songs) and it was taking up all 
physical memory and not leaving an for cache.
Shows you the swap space just isn't good enough.
If I call mpg321 or madplay with a smaller playlist then it leaves > 
10Mb free and buffers and chache take up about 25Mb. Not bad for only 
44Mb of RAM.
