Nocat Auth does the majority of this, (well it actually captures HTTP
packets and sends you to an authentication page, what you do on this
page to allow an auth pass is up to you)

I think there used to be (probably still is) a distro that has it
preconfigured (ready to customise with your auth page) but I can't
remember what it was (it was a few years ago I looked into it)
