Here is my (probably not correct, but working fine) excerpt from my
xorg.conf in the monitor section, where the refresh rates are kept:

Section "Monitor"

Note, this is the section that varies from computer to computer, monitor
to monitor.  These settings are for an extremely cheap, crappy to look
at, and crappy display quality 19" CRT.

I must admit, setting up X on a variety of different monitors with
different refresh rates, this is probably the most annoying and tedious
part.  This is mainly due to the fact that a lot of monitor manufactures
don't bother putting their Horizontal Sync and Vertical Refresh ranges
on their websites, or in their manuals, especially for older CRT's.

Hope that helps a bit, those ModeLine lines above are specific to my
monitor so you can remove them straight away :P
